Role Description
The Registered Dietitian/Health Coach utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal health status. Appointments are via telehealth or virtual only. This is a Work From Home position. The hours are M-F with one late night until 9pm EST each week.

Must have CDCES (Certified Diabetes Care and Education Specialist).

Conducts comprehensive evaluation of member’s health status and risk with use of clinical tools and information/data review.

Has knowledge and experience of lifestyle risk behaviors.

Establishes a plan of care based on member’s health status/risks, education, and support needs.

Identifies quality of care issues through established channels.

Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes.

Provides coaching and support to empower the member to make ongoing independent healthy lifestyle choices.

Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member’s goals.

Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.
Qualifications

Certified Diabetic Educator.

Registered Dietitian with licensure.

3+ years of coaching experience required.

Bachelor’s Degree in health related field (e.g., Dietetics, Health Education, Wellness, Health Promotion).

Experience in wellness related areas such as diabetes, weight management, stress management, nutrition, physical activity, or health lifestyle behavior change.

Demonstrated ability to communicate, problem solve, and work effectively with people.

Use good judgment in challenging situations.
Requirements

Health Coach Certification or Fitness certification recommended.

Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness-based and other evidence-based coaching techniques preferred.

Excellent computer skills including MS Office applications such as Excel, Word, PowerPoint, and Outlook.
