Role Description
The Registered Dietitian (RD) provides clinical nutrition services, including nutrition assessment, modified diet formulation, self-management training, and nutrition intervention to members. As the RD, you will be responsible for a variety of tasks which includes, but are not limited to:

Improving the quality of life and health literacy of members

Promoting effective healthcare utilization

Monitoring healthcare resources to achieve optimal outcomes for members

Assisting in the coordination of care and services of selected member populations across the continuum of illness

Promoting the effective utilization of healthcare resources to ensure member satisfaction and quality care
This position operates in a remote capacity with normal work hours, Monday-Friday, 8a-5p.
Duties & Responsibilities

Evaluates members for case management services and determines the appropriate level of care
-
Assesses the nutritional status and provides nutrition intervention to members, including diet education
-
Screens members for nutritional risk and malnutrition
-
Develops dietary plans of care

Provides nutrition consults/assessments as requested by team members
-
Coordinates nutritional care with other care team members including CMs, SW, NPs, and members assigned to a PCP
-
Uses clinical nutrition standards of care to assess patient nutritional needs
-
Educates and supports members’ needs, taking into account patients’ physical or age-related needs
-
Accurately charts all assessments, findings, the education given, care plan, and outcomes in the patient’s electronic chart

Collaborates with other departments to ensure members’ needs are met
-
Participates in Interdisciplinary Care Team meetings, as needed
-
Develops positive relationships with clients, caregivers, providers, and other team members
-
Ensures quality, effectiveness, and efficiency in case management activities

Complies with all HIPAA regulations
Qualifications
-
Bilingual (English/Spanish) is preferred
-
Experience in the care of the elderly in some geographic regions is preferred
-
Experience in home health, physician’s office, or public health is preferred
-
Knowledge of HIPAA regulations
-
Knowledge of CMS guidelines
Requirements
-
Bachelor's Degree (BA) in Nutrition required
-
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) Certification is required
-
Licensed in all states in which Adobe is contracted, if required by the state
