[Hiring] Referral & Intake Nurse @Unity Hospice

Role Description

At Unity Hospice, we bring peace of mind and comfort to patients and families when it matters most. We’re growing and looking for a Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories. This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.

Work environment: Remote

Schedule: Part-time working 8:00 a - 4:00 p or 9:00 a - 5:00 p, working every 4th weekend, 1-2 holidays per year.

What You’ll Do:

• Manage incoming referrals and gather patient medical information
• Communicate with hospitals, clinics, and care teams
• Coordinate initial visits with patients, families, and admissions staff
• Verify insurance coverage and obtain necessary orders
• Support eligibility review and follow up on pending referrals
• Maintain accurate and timely documentation

Qualifications

• Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
• Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage
