Role Description

We are seeking a part-time/per diem (4-5 hours per week) experienced Psychiatric Mental Health Nurse Practitioner to join our growing team. In this role, you will provide consultative psychiatric care to Bluestone Primary Care patients in coordination with primary care and provide consultation to the Bluestone Behavioral Health Integration Program.

The Psychiatric Mental Health Nurse Practitioner supports the collaborative care team—primary care provider, patient, and behavioral health care manager—through regular consultation on an assigned caseload of patients. As the consultant, the Nurse Practitioner advises the primary care provider on appropriate medications and other treatment strategies, also providing the care team with educational support on diagnosing and managing behavioral health conditions. If needed, patient visits are primarily delivered via telehealth.

Key Responsibilities

• Provide regularly scheduled (usually weekly) caseload consultation to behavioral health care managers.
• Work with behavioral health care managers to oversee the status and population health of their assigned patient caseload.
• Use a registry to track all assigned patients and prioritize patients for case review and treatment adjustment.
• Recommend any treatment adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions.
• Advise on treatment for patients who may need more intensive or more specialized mental health care.
• Provide occasional ad hoc telephone consultation to primary care providers for very complex patients or challenging patient situations.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified through ANCC
• Licensed or ability to obtain licensure(s) through the Board of Nursing.
• Ability to obtain and/or maintain DEA certification for the State(s) required for work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications

• 2+ years of clinical experience as a Psychiatric Mental Health Nurse Practitioner

Compensation

• Compensation: $105/per hour - $130/per hour
