Role Description
As Cabana launches its Collaborative Care (CoCM) program, we are seeking a Psychiatric Consultant to join our founding clinical team. This is not a traditional consulting role within an established system — it is an opportunity to help build and define Cabana’s psychiatric consultation model from the ground up.
In this remote, contract-based role, you will serve as a core clinical partner supporting Behavioral Health Care Managers (BHCMs) and Primary Care Providers (PCPs) across Louisiana. You will provide evidence-based psychiatric consultation, guide measurement-based care initiatives, and help establish the workflows and clinical standards that will shape Cabana’s integrated behavioral health programs as they scale.
This role is ideal for a psychiatrist who thrives at the intersection of clinical excellence, systems-building, and collaborative care innovation. You should be energized by helping primary care teams manage behavioral health conditions effectively while contributing to the design of a new integrated care infrastructure.
This is a contract role expecting up to 10 hours a month.
What you will do

Structured Case Consultation
-
Provide systematic psychiatric consultation to Behavioral Health Care Managers (BHCMs) through Structured Case Review (SCR), with emphasis on:

High-risk patients
-
Patients new to treatment
-
Individuals not meeting clinical targets
-
Complex diagnostic presentations
-
Advise Primary Care Providers (PCPs) regarding:
-
Evidence-based pharmacologic treatment recommendations
-
Diagnostic clarification strategies
-
Lab monitoring and medication management considerations
-
Document treatment recommendations directly within partner electronic health record systems to support seamless care coordination.

Remain available for occasional urgent consultations with PCPs regarding complex patient presentations, targeting response times within one business day (or one hour for urgent needs).

Population Health & Registry Management
-
Utilize Cabana’s clinical registry and measurement-based care systems to:
-
Track patient progress across assigned caseloads
-
Identify patients requiring treatment escalation or care adjustments
-
Support population-level behavioral health management initiatives
-
Participate in weekly caseload review meetings with BHCMs and interdisciplinary care teams.

Review validated outcomes data utilizing tools such as:
-
PHQ-9
-
GAD-7
-
Columbia Suicide Severity Rating Scale (C-SSRS)
-
Support treatment-to-target care planning and symptom remission strategies.

Provider Education & Clinical Collaboration
-
Serve as a clinical mentor and psychiatric resource for PCPs and BHCMs.

Provide guidance regarding:

Behavioral health treatment planning
-
Diagnostic conceptualization
-
Medication management strategies
-
Management of complex psychiatric presentations
-
Collaborate closely with interdisciplinary care teams to improve integration of behavioral health services within primary care settings.

Maintain strong communication and collaborative relationships across remote care teams.

Program Development & Workflow Design
-
Partner with Cabana’s Clinical Director to:

Design and refine psychiatric consultation workflows
-
Develop escalation pathways and consultation protocols
-
Establish documentation standards for CoCM implementation
-
Support development of:
-
Standard operating procedures (SOPs)
-
Clinical workflow frameworks
-
Quality assurance processes
-
Pilot program launch strategies
-
Participate in quality review meetings and contribute psychiatric insight to operational and performance improvement discussions.

Perform all other duties, as assigned.
Qualifications
-
MD or DO with an active, unrestricted Louisiana medical license
-
Board Certified in Psychiatry (ABPN or equivalent)
-
Minimum of 3+ years of clinical experience, preferably in one or more of the following:
-
Integrated behavioral health
-
Primary care behavioral health
-
Consultation-liaison psychiatry
-
Collaborative Care (CoCM) environments
-
Strong knowledge of evidence-based psychiatric medication management for common behavioral health conditions, including:
-
Anxiety disorders
-
Depression
-
PTSD
-
Behavioral health conditions with medical comorbidities
-
Experience utilizing measurement-based care tools, including:
-
Experience working within electronic health record (EHR) systems, such as Epic, Cerner, or other equivalent EHR platforms.

Strong written communication and clinical documentation skills.
-
Ability to thrive in a fast-paced, evolving startup environment with changing workflows and priorities.

Must reside within the United States while maintaining active Louisiana medical licensure.
Requirements

Prior experience working within the Collaborative Care Model (CoCM)
-
Familiarity with “treat-to-target” protocols and registry-based population health management
-
Experience consulting within:
-
Primary care settings
-
Federally Qualified Health Centers (FQHCs)
-
Integrated care environments
-
Familiarity with CoCM and Behavioral Health Integration billing requirements, including CPT codes: 99492, 99493, 99484
-
Comfort managing population-based caseloads rather than traditional appointment-only psychiatric models.

Strong collaboration and interdisciplinary communication skills.

Demonstrated ability to build workflows and contribute to clinical program development initiatives.

High level of adaptability and comfort operating in early-stage program environments.
