Role Description

Spoon River Home Health is looking for a compassionate and dependable Registered Nurse (RN) to join our team on a PRN basis, providing skilled nursing visits to patients in Peoria and Tazewell counties. This is a hands-on, field-based home health position for an RN who enjoys working independently and providing one-on-one care in patients' homes. We are specifically seeking an RN with Monday and Friday availability.

Compensation:

• Patient visits are compensated on a per-visit basis, with rates varying based on the type of visit performed.
• Applicable non-visit work, such as required nursing meetings, is compensated at an established hourly rate within a $35-45 range.

What You'll Do:

• Travel to patients' homes throughout Peoria and Tazewell counties.
• Provide skilled nursing care based on patient needs and physician orders.
• Complete patient assessments and ongoing evaluations.
• Perform skilled nursing visits and required clinical documentation.
• Monitor changes in patient condition and communicate with physicians and the care team.
• Educate patients and families regarding medications, treatments, diagnoses, and care needs.
• Collaborate with supervisors, therapists, physicians, and other members of the interdisciplinary care team.
• Follow agency policies, clinical standards, and regulatory requirements.

Qualifications

• Current Illinois RN license.
• Minimum of 1 year of nursing experience.
• Availability on Mondays and Fridays.
• Strong clinical assessment and critical-thinking skills.
• Excellent time management and organizational skills.
• Comfortable working independently and traveling to patients' homes.
• Dependable, self-motivated, and compassionate.
• Strong communication and documentation skills.
• Home health experience preferred, but not required.
