Role Description
Join our Virtual Rehabilitation Team to deliver evidence-based physiotherapy assessment and treatment to patients receiving rehabilitation care remotely and in-person across the southern Adelaide metro area.
Reporting to the Virtual Rehabilitation Ward Manager, this role is integral to providing care to older people with complex medical, functional and/or psychosocial needs in their own home (which includes residential aged care settings) in lieu of an extended or avoidable hospital admission.
Duties for the role will include:

Contribute to safe and timely discharge and improved independence and physiotherapy outcomes for clients.

Collaborating with nursing, medical, and allied health colleagues to coordinate patient care across the care continuum.

Maintaining accurate clinical documentation in line with SA Health standards and contributing to service improvement activities.

Participating in, and where appropriate, providing education and staff development within the team.

Participating in staff meetings and contributing to planning and policy development for Physiotherapy services.

Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.
Qualifications

A degree or equivalent qualification which entitles registration as a Physiotherapist with the Physiotherapy Board of Australia.

Experience in the provision of clinical Physiotherapy services to clients with a range of illnesses and disabilities relevant to older people and rehabilitation.

Ability to undertake assessment and intervention with clients with a range of illnesses and disabilities relevant to older people and rehabilitation.

Ability to provide clinical services as a member of a multidisciplinary team.

Ability to communicate with clients and a range of professional and other staff, orally and in writing, including in the provision of supervision to Allied Health Assistants and volunteers as required.

Organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately and demonstrate initiative.

Appointment will be subject to a satisfactory Criminal History Check and Immunisation compliance as indicated on the Role description.
