Role Description
As a Fully Remote Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity-related conditions as appropriate in a virtual primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
Schedule: Monday-Friday with flexible scheduling (32 patient contact hours minimum + 8hrs of administrative time). Fully Remote. No scheduled evening/weekend responsibilities outside of phone On-Call.
On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24). 24/7/365 physician back up.

Responsibilities:

Practice exceptional medicine within knownwell.

See adults for metabolic health services as is appropriate in a virtual setting.

Embodies the knownwell mission and values of patient inclusivity in how you deliver care.

Coordinates with the broader care team to ensure the patient has the support they need.

Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.

Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.

Ensure panel is open to any individual, regardless of their current health status and comorbidities.

Listen to the aspirations of the patient and ensure care is tailored to the patientβs unique health journey.

Coordinate across the collaborative care team.

Escalate any concerns to your dedicated site leadership team.

Provide clinical guidance to medical assistants and nursing teams as assigned.

Stay current with licensure and appropriate re-certifications.
Qualifications

Master's degree in Physician Assistant studies certification by the Physician Assistant certifying body (NCCPA), with 10+ state licensure and willingness to obtain additional as needed.

Active license in Missouri required.

Active, unrestricted licenses to practice in 5+ states in addition to MO, and a willingness to obtain additional as needed by the organization. Preferred states include CA, TX, FL, NY, PA, IL, OH, GA, NC, MI.

Unrestricted DEA licensure and prescribe authority (per state practice regulation).

At least 4 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
