Role Description
As a Physical Therapist with Best Home Health Providers, you will play a vital role in delivering skilled care that addresses the unique challenges of home health environments, helping patients regain mobility, independence, and confidence.

Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.

Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.

Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.

Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.

Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.

Manage documentation including clinical progress summaries and discharge planning.

Identify needs for additional home health services and coordinate referrals as appropriate.

Perform other duties as required for patient services.
Qualifications

Bachelor’s or Master’s degree in Physical Therapy from an accredited program.

Active California Physical Therapy license.

Minimum 2 years of PT experience; home health background is a plus.

Strong communication skills and the ability to connect with patients and families.

Current CPR certification with AHA.

Valid driver’s license and reliable, insured vehicle.
