Role Description

The VISN 8 Clinical Contact Center Pharmacy Service is seeking an experienced and dynamic overnight Pharmacist-Clinical Pharmacy Specialist. Ideal applicants are those who want to contribute to the achievement of quality care standards, as well as the growth and development of a VA academic hospital program. You must be an excellent communicator and team player who is able to perform all duties required of pharmaceutical care.

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties include but are not limited to:

• Implementation and monitoring of therapeutic drug plans.
• Obtain medication histories from patients and summarize significant findings in the medical record.
• Serve as a provider with prescribing privileges as outlined in the individual's Scope of Practice.
• Review Patient medical record and interview patients to detect drug-related problems.
• Provide patient education about medications and related topics when appropriate.
• Assess patients for drug therapy needs.
• Serve as Pharmacist in charge in the absence of Pharmacy supervisor.
• Conduct drug utilization review, quality assurance activities, and utilization reviews as required.
• Document clinical interventions in CPRS in a timely and professional manner.
• Promote and monitor compliance for established drug therapy policy.
• Provide excellent customer service to Veterans, caregivers, and internal customers.
• Ensure the V-IMPACT Hub Pharmacy Program is in compliance with all regulations and accreditations.
• Assist in the coordination of V-IMPACT Hub Clinical pharmacy service including training residents and telehealth pharmacist.

Work Schedule: 7 days on/7 days off (Overnight); from 7:00pm to 7:00 am.

(Note: Work schedules are determined and approved by the supervisor. The incumbent may be required to work various shifts on a permanent or temporary basis based on facility/patient care needs).

Qualifications

• Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
• United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
• Education:
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or Doctor of Pharmacy (Pharm.D.) degree.
  • Graduate of foreign pharmacy degree meet the educational requirements if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
• Licensure: Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico), or the District of Columbia. Must maintain current registration.

Requirements

• In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
• These duties include but are not limited to reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions.

• This position requires moderate lifting up to 44 lbs., carrying objects up to 15 lbs., frequent reaching above shoulder, use of fingers, hands and legs, use of cranes and motor vehicles, walking & standing up to 8 hours, bending and climbing and able to hear whisper.
• The incumbent must be physically, emotionally and mentally stable in order to efficiently perform the essential functions of this position without hazard to themselves or others.
