[Hiring] Patient Access and Intake Coordinator @Kindred Healthcare

Role Description

The Patient Access & Intake Coordinator plays a key role in facilitating a smooth and efficient patient admission process. This position provides vital administrative and operational support to the Business Development team and the Central Access and Authorizations Team (CAAT), ensuring timely initiation and management of referral activity.

• Responsible for launching the referral management process, overseeing admission workflows, gathering and verifying required documentation, coordinating information for necessary approvals, and communicating essential information to relevant departments.
• Acts as a central liaison between Business Development, facility leadership, and CAAT colleagues, coordinating admission logistics, confirming readiness to receive the patient, and ensuring all clinical and operational details are aligned prior to arrival.
• Contributes to quality improvement, problem solving, and productivity initiatives within an interdisciplinary team model, demonstrating accountability and a commitment to operational excellence.

Qualifications

• Team player, able to communicate and demonstrate a professional image/attitude.
• Excellent oral and written communication and interpersonal skills.
• Strong computer skills with both standard and proprietary applications.
• Data entry with attention to detail.
• Must read, write, and speak fluent English.
• Must have good and regular attendance.

Requirements

• High School Diploma or GED required.
• Associates or Bachelors Degree preferred, with a preference towards a healthcare related area of concentration or being a licensed health care provider or equivalent experience.
• Experience in a healthcare related area is preferred.
• Work experience in post-acute care a plus.
