Role Description

We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.

• Lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure.
• Adhere to evidence-based guidelines for type 2 diabetes and hypertension.
• Oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider.
• Deliver timely, patient-centered, and scalable remote care.
• Work in a highly collaborative, fast-paced clinical role with clear expectations for productivity, responsiveness, and quality.

Qualifications

• Active Compact Multi-State RN license.
• Active NP licensure in CA, WA, or AZ preferred.
• 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D).
• 3+ years of cardiology experience with a focus on heart failure management and GDMT.
• Active ANCC or AANP certification.
• Master’s Degree as a Nurse Practitioner.
• Home office setup with consistently stable Wi-Fi.
• Experience in fast-paced environments.
• Experience working in a CHF bridge clinic or T2D clinic preferred.

• Comfortable receiving frequent feedback and continuously adapting processes.
• Highly organized with excellent time management skills.
• Self-directed and accountable while working remotely with minimal supervision.
• Curious, resilient, and energized by helping build new ways of delivering care.
• Comfortable using AI-assisted clinical tools while maintaining clinical judgment.
