[Hiring] Nurse Behavioral Health Field Care Manager @Humana

Role Description

The Field Care Manager Nurse 2 assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members.

• Employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
• Identifies and resolves barriers that hinder effective care.
• Ensures patients are progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
• May create member care plans.
• Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.

You will report to the Manager, Care Management.

Qualifications

• Bachelor's in nursing (BSN) and have an active license in the state of South Carolina without disciplinary action.
• Must reside in the State of South Carolina.
• 2 or more years of experience in case/care management.
• Completed a minimum of 45 documented training hours related to Behavioral Health issues and treatment.
• Knowledge of community health and social service agencies and additional community resources.
• Use a variety of electronic information applications/software programs including electronic medical records.

Requirements

• 3 or more years of in-home assessment and care coordination experience (preferred).
• Experience with health promotion, coaching, and wellness (preferred).
• Previous managed care experience (preferred).
• Bilingual — English, Spanish (preferred).
• Certification in Case Management (preferred).
• Motivational Interviewing Certification or knowledge (preferred).
• Field workstyle.
• Travel: will be 50% of the time to support collaboration, conduct face-to-face meetings, and engage directly with staff, providers, members, and their families.
• Schedule: Monday through Friday 8:00 AM - 5:00 PM Eastern Time.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.

$71,100 - $97,800 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
