Role Description

This role will manage the Operation Integration Specialists (OIS) in the administrative and clinical practice transformation programs and alternative payment models (APM) leading to an advanced level patient-centric approach that allows the organization to thrive in a value-based health care model.

  • Proactively maintain a thorough understanding of the requirements and regulations of the alternative payment practice transformation programs (i.e., CPC+, Ohio CPC, PCF etc.)
  • Provide guidance and support to team members who provide practice level guidance and assist to drive continuous quality improvement efforts through process redesign and performance management.
  • Ensure accountability for accurate performance of processes and workflows.
  • Support the development and maintenance of high performing teams.

Essential Job Functions

  • Effectively educate the OIS team on the elements, measures, tasks, and tools to support various practice transformation functions and activities.
  • Act as a professional role model and provide leadership in motivating and inspiring the team.
  • Maintain thorough working knowledge of all models of care, data elements, and tools/resources to support a high functioning team.
  • Act as a liaison between the OIS team and population health team, including monitoring and appropriately elevating any potential issues, challenges, barriers, and success stories with leadership.
  • Manage activities of the OIS team members including rounding schedules, communications, issue resolution, workflows for onboarding new providers, and complying with evolving government reporting requirements.
  • Develop and maintain the team plan to reflect necessary resources, tasks, and overall timeline needed to complete program requirements.
  • Ensure the OIS team supports, encourages, and works closely with providers and their care teams to implement process & quality improvement activities.
  • Monitor and elevate any potential issues, challenges, or barriers and communicate escalation as appropriate.
  • Educate and communicate with providers as the primary resource for alternative payment practice transformation programs.
  • Manage the collection, analysis, translation, and reporting of quality and performance metrics.
  • Work closely with the data analytics team and EPIC EHR team to obtain accurate and actionable information/data.
  • Direct activities to collect, track, and analyze practice demographics and transformation activities.
  • Attend required training, education, and learning events by webinar, teleconference, virtual or in person.
  • Act as the subject matter expert (SME) on government alternative payment models.
  • Perform other duties as assigned.

Qualifications

  • Bachelors in Healthcare Administration or nursing related field
  • Masters in Healthcare Administration or nursing related field (preferred)

Requirements

  • 5 years of experience in an ambulatory healthcare setting, preferably in a leadership role in a highly matrixed organization.
  • Subject Matter Expert in physician practice operations and practice transformation activities.
  • Strong analytical skills.
  • Ability to manage multiple simultaneous projects.
  • Work independently and proactively in a stressful medical environment.
  • Attentive to details and accuracy.
  • Able to manage time, priorities, and resources.
  • Strong verbal, written, and interpersonal skills.
  • Experience and working knowledge of Microsoft Office (Word, Excel, PowerPoint).
  • Ability to travel locally and regionally.
  • Ability to plan, hold, and facilitate meetings and networking events.
  • Working knowledge of EHR’s, preferably EPIC.
  • Familiarization with process improvement models (such as PDSA, A3, CQI, Lean, Six Sigma).
