Role Description
As a LPN Clinical Liaison, you will be a critical member of the clinical care team, providing both clinical and administrative support to ensure high-quality, efficient member care. This role assists providers by:

Accurately documenting patient encounters in Thyme Box and supported electronic health records (EHR).

Supporting patient flow and coordinating care activities.

Maintaining complete and accurate medical records.

Improving clinical workflow while ensuring timely communication and continuity of care.
Qualifications

At least 2 years of experience as a LPN, especially with telehealth experience.

Experience working at a startup or tech-forward company is exceptional.

Compact LPN license.
Requirements

Complete training and be up to speed on Thyme Careโs systems, tools, technology, partners, and clinical expectations within the first three months.

Maintain patient confidentiality in accordance with HIPAA regulations.

Accurately document patient/member encounters after visitsโwithin Thyme Box and other supported EHRs, capturing histories, assessments, treatment plans, orders, diagnostic results, referrals, prescriptions, and follow-up instructions.

Organize patient charts and gather relevant clinical information ahead of visits, including vital signs, medication reconciliation, and other required data.

Review member eligibility for services and verify insurance/coverage.

Facilitate referrals, follow-up appointments, and diagnostic testing, keeping care plan information updated to ensure continuity of care.

Conduct chart reviews across multiple electronic platforms to consolidate and streamline transitions of care information.

Monitor shared clinical inboxes and electronic communications, triaging and routing messages promptly to the appropriate provider, department, or team.

Coordinate and communicate with internal and external healthcare partners to support seamless and coordinated care.
