[Hiring] Liaison/Triage Practitioner @Psychiatry UK

Role Description

We're looking for compassionate and experienced Liaison/Triage Practitioners to join our established Liaison Nurse Team (LNT). In addition to supporting our well-established adult services and our recently launched North East and North Cumbria (NENC) Neurodevelopmental Triage Service, successful candidates will play an important role in the continued delivery and development of the Liaison Nurse Service as it expands to support our forthcoming Children and Young People (CYP) Right to Choose pathways.

This is a varied clinical role where you'll work across both the Liaison Nurse Team (LNT) and the North East and North Cumbria (NENC) Neurodevelopmental Triage Service.

• Provide specialist mental health support across both adult and Children and Young People (CYP) services.
• Undertake clinical reviews, assess and manage risk, support safeguarding processes, provide clinical advice and liaison.
• Help ensure patients receive the right care, at the right time, through the most appropriate pathway.
• Undertake holistic neurodevelopmental triage and risk assessments for adults referred with possible neurodevelopmental conditions.
• Consider neurodevelopmental presentations, mental health needs, safeguarding concerns and functional impact to support timely access to the most appropriate assessment, treatment or care pathway.

Qualifications

• Registered Nurse (NMC) or Occupational Therapist (HCPC) with current professional registration.
• A minimum of three years’ post-qualification experience in a relevant mental health setting, or demonstrable equivalent experience of safe, independent clinical decision-making within mental health services.
• Evidence of ongoing Continuing Professional Development (CPD).
• Relevant experience in mental health care and/or neurodevelopmental conditions.
• Experience of working with Children and Young People (CYP) within CAMHS or other CYP mental health services.
• Experience in the treatment and management of mental health conditions.
• Ability to undertake holistic clinical triage and risk assessments and apply evidence-based clinical decision-making.
• Sound understanding of safeguarding, functional impairment and the impact of co-occurring mental health conditions.
• Ability to work autonomously, prioritise effectively and manage your own workload.
• Strong digital and IT skills appropriate to remote working and online service delivery.
• Evidence of effective multidisciplinary and multi-agency working.
• Good working knowledge of relevant legislation, professional guidance and developments relating to ADHD, autism, and adult and CYP mental health services.
• Enhanced DBS clearance.

Requirements

• Provide clinical support across adult services, Children and Young People (CYP) pathways, and the North East and North Cumbria (NENC) Neurodevelopmental Triage Service, adapting to service demand and organisational priorities.
• Provide expert clinical support and advice to Psychiatrists, Prescribers and the wider multidisciplinary team.
• Assess patients' mental state, clinical presentation and level of risk of children, young people and adult patients through telephone and video consultations.
• Deliver timely clinical triage and risk assessments in line with Psychiatry UK policies and agreed care pathways.
• Triage referrals for neurodevelopmental assessments using evidence-based clinical and functional criteria.
• Make autonomous clinical decisions, ensuring safe, effective and person-centred care.
• Develop and implement appropriate risk management plans, escalating concerns and referring to external or emergency services where required.
• Maintain accurate, timely and comprehensive clinical records within electronic patient systems.
• Promote shared decision-making and provide patients, families and carers, where appropriate, with information, advice and support regarding assessment, treatment and onward care.
• Work collaboratively with internal teams, NHS partners and external agencies to ensure coordinated patient care.
• Contribute to the development, evaluation and continuous improvement of clinical services and care pathways.
• Undertake safeguarding assessments and ensure appropriate action is taken in line with organisational policies.
• Prioritise and manage a varied clinical workload while maintaining high standards of care and professionalism.
• Maintain up-to-date knowledge of relevant legislation, national guidance, evidence-based practice and professional standards.
• Participate in clinical supervision, mandatory training and continuing professional development to maintain professional competence.
