Role Description

Presbyterian Medical Services (PMS) is seeking a dedicated Part-Time Behavioral Health Intake Coordinator (20 hours per week) to serve as the first point of contact for individuals seeking behavioral health services. In this role, you will:

• Conduct comprehensive psychosocial and diagnostic assessments.
• Provide crisis intervention and coordinate referrals.
• Connect clients with appropriate treatment and community resources.
• Work closely with clients, families, providers, and community partners.
• Assess needs, establish medical necessity, and support continuity of care.

Qualifications

• Minimum of Master’s degree from an accredited college or university.
• Current independent New Mexico licensure as an LPCC, LISW, LCSW, LMFT, or LPAT.
• Minimum of three years of experience in the mental health treatment field.
• At least one year of assessment, intake, or triage experience.
• Bilingual English/Spanish (preferred).

Requirements

• Conduct comprehensive intake assessments, including mental status examinations and diagnostic evaluations.
• Determine medical necessity and appropriate levels of behavioral health care.
• Provide crisis intervention, triage services, and short-term support during urgent situations.
• Coordinate referrals to internal and external behavioral health and community resources.
• Serve as the primary point of contact for new client inquiries and service requests.
• Complete intake screenings, registration processes, and required documentation.
• Provide active case management and follow-up to ensure successful connection to services.
• Collaborate with interdisciplinary teams to coordinate care and support positive client outcomes.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in outreach, community education, and performance improvement activities.
