Role Description

The Health System Command Center Pharmacy Coordinator supports centralized pharmacy operations through a virtual command center model that serves multiple hospitals and ambulatory care settings across the health system.

• Assists pharmacists and clinical care teams by performing medication reconciliation, patient outreach, medication access coordination, and prior authorization support.
• Ensures accurate medication management during care transitions.
• Supports Hospital-at-Home programs, facilitating medication coordination, patient communication, and medication access services to ensure safe medication use outside the hospital environment.
• Functions as part of a remote pharmacy services team designed to improve medication safety, patient outcomes, and care continuity across the health system.
