[Hiring] Field Care Manager, Behavioral Health @Humana

Role Description

The Field Care Manager, Behavioral Health assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

• Employ a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.
• Identify and resolve barriers that hinder effective care.
• Perform telephonic and face-to-face assessments and evaluations of the member's needs.
• Ensure member is progressing towards desired outcomes by continuously monitoring care.
• Create member care plans and understand department, segment, and organizational strategy.
• Collaborate with providers and community services to promote quality and cost-effective outcomes.
• Ensure the delivery of needed services/supports for Behavioral Health, Physical Health, Social Determinant of Health, and value-added benefits.
• Coordinate across the transdisciplinary care team and transitions of care.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
• Submit incident reports.

Qualifications

• Must reside in Louisiana Region 7 (Bienville, Bossier, Caddo, Claiborne, DeSoto, Natchitoches, Red River, Sabine, Webster).
• LCSW, LPC, LAC, LMFT, or RN with a minimum of 2 years of experience in psych.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Preferred Qualifications

• Bilingual a plus.

Requirements

• This role is considered patient-facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
• Section 1121 of the Louisiana Code of Governmental Ethics states that current or former agency heads or elected officials, board or commission members, or public employees of the Louisiana Health Department (LDH) who work directly with LDH’s Medicaid Division cannot be considered for this opportunity. A separation of two (2) or more years from LDH is required for consideration.
• Work at Home Requirements: Self-provided internet service must meet a minimum download speed of 25 Mbps and an upload speed of 10 Mbps; a dedicated space lacking ongoing interruptions is required to protect member PHI / HIPAA information.
• Travel: Occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.

$65,000 - $88,600 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
