Role Description

This is a Field Based role with a Home-Based office. If you are located in or within commutable driving distance to Brownsville, TX, you will have the flexibility to work remotely as you take on some tough challenges.

• Assess, plan and implement care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care.
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services.
• Manage the care plan throughout the continuum of care as a single point of contact.
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
• Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team.

Qualifications

• Current Registered Nurse unrestricted license in the state of Texas (no compact license).
• 2+ years of experience working within a healthcare role.
• Intermediate level of proficiency with Microsoft Office, including Word, Excel, and Outlook.
• Valid driver’s license, access to reliable transportation, ability to travel up to 75% in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' office.
• Reside in or within commutable driving distance to Brownsville, TX.

Requirements

• 2+ years of experience working within the community health setting (preferred).
• 1+ years of experience with long term care services and support, Medicaid or Medicare (preferred).
• Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations (preferred).
• Demonstrated ability to create, edit, save and send documents, spreadsheets and emails (preferred).
