Role Description

The Executive Medical Advisor serves as the senior physician strategist and clinical advisor for OneTogether Solutions, shaping the integrated practice model through which OTS delivers sovereign, whole-person care to the communities it serves. Operating under the Tribal 638 self-determination model in partnership with federally recognized tribes, OTS treats Opioid Treatment Program services as the foundation of a continuous care relationship that opens onto urgent care, dental care, behavioral health, housing support, and the broader social determinants of wellness.

The Executive Medical Advisor carries administrative responsibility for the Site Medical Directors across the OTS supported clinics, including:

• Administrative oversight of program participation
• Performance of administrative duties
• Professional development
• Monitoring provider productivity and staffing levels
• Adherence to system-wide clinical infrastructure

Site Medical Directors retain full independent clinical authority over patient care and clinical decision-making at their respective sites, consistent with applicable Corporate Practice of Medicine requirements. The role is structured as full-time, 30 hours a week, with flexible structure of work.

Qualifications

• MD or DO degree from an accredited medical school
• Board certification in Addiction Medicine (ABPM) or Addiction Psychiatry (ABPN), or board certification in a primary specialty (Family Medicine, Internal Medicine, Psychiatry, Emergency Medicine) with Addiction Medicine subspecialty certification
• Active, unrestricted medical license in at least one state with willingness to obtain additional state licensure if required
• Active DEA registration in good standing
• Minimum of five years of direct clinical experience in an Opioid Treatment Program (OTP) setting, with demonstrated expertise in methadone and buprenorphine treatment for opioid use disorder
• Minimum of three to five years of progressive medical leadership experience, including roles such as Medical Director, Associate Medical Director, or comparable physician advisory or executive positions
• Demonstrated experience designing or leading integrated clinical programs that bridge addiction treatment with primary care, behavioral health, or other adjacent services
• Demonstrated experience leading clinical quality, peer review, and accreditation processes

Preferred Qualifications

• Prior experience serving in a senior physician leadership role within a multi-site OTP, federally qualified health center, Indian Health Service facility, tribal health program, or integrated addiction care delivery system
• Experience working with tribal health programs operating under the Indian Self-Determination and Education Assistance Act (Public Law 93-638) or with IHS-funded care delivery models
• Master's Degree in Public Health, Healthcare Administration, or Business Administration
• Experience with EHR systems used in behavioral health and addiction medicine settings, such as NextGen, Methasoft, Smart Management, or Welligent
• Demonstrated cultural humility and experience providing culturally responsive care to American Indian and Alaska Native communities
• Working familiarity with the Corporate Practice of Medicine doctrine and physician practice management structures in multi-state operating environments

• Comfort operating in a growth-stage, multi-state, multi-stakeholder environment with attention to Corporate Practice of Medicine principles

Key Responsibilities

• Integrated Clinical Strategy and Program Design: Set the system-wide clinical vision and design the integrated practice model that positions OTP as the foundation of a continuous patient relationship.
• Site Medical Director Leadership and Medical Staff Development: Carry administrative responsibility for Site Medical Directors across the OTS system.
• Clinical Program Infrastructure: Design system-level clinical workflow tools and develop clinical policies, protocols, and order set templates.
• Regulatory, Accreditation, and Compliance Strategy: Provide clinical strategy and subject matter expertise to ensure OTS's clinical infrastructure aligns with all applicable federal, state, and tribal regulatory requirements.
• Quality, Safety, and Governance: Chair the Peer Review Committee and conduct monthly random independent sample audits of chart documentation.

• Preferred Location: Washington State, California, or Colorado
• Travel Requirements: 30-40% Travel to Clinics, OTS HQ, & Industry Event
• Salary: $300,000 - $300,000 a year
