Role Description

Are you a passionate and motivated Physiotherapist looking to make a real difference to the lives of people living with cardiac conditions? We are seeking an enthusiastic Community Cardiac Rehabilitation Physiotherapist to join our established and supportive Cardiac Rehabilitation Team. This is an exciting opportunity to lead and deliver high-quality, evidence-based cardiac rehabilitation programmes, supporting patients to improve their health, wellbeing and quality of life following a cardiac event or diagnosis.

• Hold responsibility for the assessment, planning, implementation and evaluation of cardiac rehabilitation interventions for patients with a variety of cardiac conditions.
• Lead and deliver cardiac rehabilitation exercise programmes in both community and clinical settings.
• Support the provision and continued development of a high-quality, patient-focused and evidence-based cardiac rehabilitation service.
• Work in line with the British Association for Cardiovascular Prevention and Rehabilitation (BACPR) Standards and Core Components.
• Collaborate with colleagues across the multidisciplinary team to optimise patient outcomes and promote health and wellbeing.
• Provide education, advice and support to patients and their families, encouraging self-management and informed decision-making.
• Adopt a flexible, professional and dynamic approach to service delivery, supporting the wider cardiac rehabilitation service as required.
