Role Description
The Clinical Pharmacist plays a pivotal role in the healthcare team, providing essential pharmacy services that contribute to optimal patient outcomes. Reporting directly to the Specialty Pharmacy Director, this position involves evaluating pharmacotherapy to enhance treatment efficiency, effectiveness, and cost-effectiveness. Key responsibilities include:

Monitoring drug therapy

Ensuring compliance with regulatory standards

Delivering patient education to ensure safe and effective medication use

Blending direct patient care with interdisciplinary collaboration

Participating in quality improvement initiatives and leadership opportunities

Reviewing and optimizing medication regimens

Addressing drug-related issues and supporting transitions of care

Contributing to institutional goals while maintaining a strong focus on patient-centered care and clinical excellence
Principal Duties and Responsibilities:

Oversee therapy management in accordance with organization policies and protocols, identify and address medication errors and adverse reactions

Consistently demonstrate the ability to respond to changing situations in a flexible manner to meet current needs assigned by the Specialty Pharmacy Director or hospital leadership

Complete appropriate documentation promptly, following Client policies and procedures

Contribute to the achievement of Quality Improvement and participate in QI activities
-
Ensure adherence to established standards, client policies, and Visante procedures. Report and resolve safety issues and incidents promptly

Perform other duties as assigned by the Specialty Pharmacy Director, Visante Leadership team, or designee

Provide appropriate medication therapy and address patient concerns confidentially

Support medication use processes to ensure safety, efficacy, and compliance with guidelines
-
Identify and resolve potential drug-related issues to optimize patient outcomes
-
Maintain accurate records of clinical interventions, adverse drug events, and quality improvement activities
-
Participate in initiatives to enhance medication safety, operational efficiency, and cost-effectiveness

Work with interdisciplinary teams to support effective patient care and program development

Respond to evolving departmental needs and contribute to various projects or initiatives as assigned
Qualifications

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy (Required)

Successful completion of an ASHP accredited PGY1 or specialty residency (Preferred)
-
3 years of retail or specialty pharmacy experience (Preferred)

Current Pharmacist license as granted by the appropriate state licensing authority obtained within 2 months of hire date (Required)

American Heart Association BLS certification obtained within 6 months of hire date (Required)

Board of Pharmacy Specialties certification (Preferred)

Strong understanding of pathophysiology and pharmacotherapeutics

Ability to interpret and apply regulatory standards

Demonstration of good judgement, multi-tasking, and meeting deadlines with a sense of urgency

Strong client relationship, interpersonal, and team skills

Excellent presentation skills with the ability to make effective presentations to all levels of management

Proven ability to diagnose and resolve issues, demonstrating strong analytical and creative skills

Ability to make sound, timely decisions based on analysis, experience, and judgement

Clear and concise verbal and written communication skills and the ability to advise clients professionally and positively
