Role Description

The Cancer Center Triage Team is a specialized nursing role dedicated to providing professional and clinical support in the planning, coordination, and facilitation of patient care for all Stanford Health Care (SHC) oncology patients and their caregivers throughout the care continuum. As an integral member of the interdisciplinary care team, the Cancer Center Triage Team will collaborate closely with:

• Physicians
• Advanced Practice Providers (APPs)
• Specialty-based Clinical Nurses
• Research Coordinators
• Inpatient teams
• Schedulers

The team ensures smooth transitions from inpatient to outpatient care, focusing on:

• Preventing unnecessary emergency department visits
• Reducing hospital readmissions
• Minimizing hospital length of stay
• Enhancing access to specialized same-day resources and services

Responsibilities include:

• Expedited symptomatic clinical triage
• Clinical advice
• Coordinating patient communication
• Patient and care provider education
• Preparing patients and/or caregivers for procedures and direct admission
• Inpatient-outpatient discharge care coordination
• Same-day access scheduling
• Healthcare navigation guidance
• Collaboration with multiple care partners to streamline and enhance the cancer patient care experience

The position is primarily a fully remote role within the SHC Cancer Center DSL, with occasional on-site requirements communicated in advance.

Qualifications

• Bachelor of Science in Nursing (BSN) from an accredited college or university preferred
• 2 years required but 5 years preferred of progressively responsible and directly related registered nurse (RN) work experience in outpatient ambulatory oncology experience required
• OCN certification preferred
• Prior telephonic oncology triage and care coordination experience preferred

Requirements

• American Heart Association Basic Life Support (BLS) certification
• California Registered Nurse (RN) license
