[Hiring] Care Manager @Tri-County Care

Role Description

The role of the Care Manager is to work collaboratively with individuals with intellectual/developmental disabilities, their families, OPWDD, and relevant providers to ensure that all individuals supported are able to live safe, happy, and fulfilling lives. This includes:

• Identifying goals
• Building plans to reach those goals
• Assisting with applying to a variety of supports and services
• Advocating for individuals with their schools
• Helping them to find medical providers
• Assisting them with applications to Social Security, SNAP benefits, Waiver programs, etc.

Qualifications

• Excellent communication skills (both written and verbal)
• Conflict-management and problem solving skills
• Computer software skills, particularly skills with Microsoft programs (Word, Excel, Outlook)
• Ability to multi-task, organize, schedule, and utilize time well
• Capability to analyze situations accurately, prioritize, and take effective action

Requirements

• A bachelor's degree with two years of relevant experience, OR
• A License as a Registered Nurse with two years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
• A master's degree with one year of relevant experience
• MSC Service Coordinators prior to July 1, 2018 are "grandfathered" to facilitate continuity of care
