Role Description

Sentara Health Plans is hiring a Care Coordinator/ Licensed Practical Nurse (LPN) for LOCERI/LTSS Waiver - Remote in Virginia.

• Support the Case Management team by reviewing completed LOCERI (annual assessments) to ensure accurate scoring and that all required elements are completed.
• Maintain a productivity target of 12–15 reviews per day.

Qualifications

• Associate's degree in Nursing preferred.
• Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.
• Licensed Practical Nurse (LPN) or Bachelor's prepared health professional strongly preferred.

Requirements

• LPN license in VA or Compact/Multistate preferred.
• 3 years Managed Care experience REQUIRED.
• Case Management experience preferred.
• Long-Term Services and Supports (LTSS) waiver experience preferred.
