Role Description

Associate Discharge Planner- REMOTE position. This position is Per Diem and will work 8-hour, Day shifts.

Under the general supervision of the Director, the Associate Discharge Planner supports the care management team by:

• Coordinating discharge planning
• Assisting with the operational functions of the department
• Ensuring efficient transitions for patients from hospital to home or alternative care settings

Qualifications

• Clinical or administrative experience in healthcare industry
