[Hiring] Advanced Complex Care Practitioner @Humber Teaching NHS Foundation Trust

Role Description

An exciting opportunity has arisen for a dynamic individual to join the Virtual Frailty Ward as an Advanced Complex Care Practitioner, providing care across Scarborough, Whitby and Ryedale. We are looking for a skilled, organised, and dynamic Nurse - AHP- with community experience to recruit as an Advanced Complex Care Practitioner (ACCP).

You will work in an integrated multidisciplinary team, including community nursing, therapies, and closely with acute care, social care, GPs, and voluntary sector colleagues to deliver individualised, safe and effective, high-quality care at home.

• Proactively identify service users' needs.
• Quickly identify a deteriorating patient.
• Have knowledge and experience of caring for those with complex long-term conditions within their home environment.
• Support patients to remain at home or in their normal place of residence, avoiding unnecessary hospital admissions.
• Facilitate early discharge for hospitalised patients.
• Work as an autonomous practitioner and collaborate with colleagues across various professions and organisations.

Qualifications

• Be a member of a recognised professional body and hold a current professional registration.
• Educated to Masters Level or Post Graduate advanced clinical/professional qualification or equivalent knowledge and experience.
• An accredited leadership qualification or advanced leadership and management skills.
• Evidence of project work impacting practice and demonstrating higher-level communication/planning/change.
• Prepared to undertake or hold a recognised non-medical prescribing qualification (desirable).
• Advanced/expert understanding of relevant clinical practice, standards, and audits (desirable).

• Evidence of sound post-registration professional practice.
• Evidence of continuing professional development (CPD) relevant to the clinical area.
• Evidence of promoting/supporting active user/carer involvement/participation.
• Leadership/management experience creating positive change within service delivery/practice.
• A breadth of clinical and professional practice including clinically specialising in the field (desirable).
• Proven leadership/management experience demonstrating positive impact/change (desirable).
• Ability to work across team/organisational boundaries (desirable).
