[Hiring] Home Care Intake Coordinator @UnityPoint Health

Role Description

This is a remote position within the geographic footprint of UnityPoint Health.

Shift: Monday - Friday | 8:00 AM - 5:00 PM

• Be the first point of contact for patients, families, and healthcare providers.
• Coordinate referrals, enter accurate patient data, and ensure smooth admissions for home health, hospice, and infusion services.
• Work in a fast-paced, collaborative environment where your attention to detail and customer service make a real impact every day.

Qualifications

• Education: High school diploma or GED
• Experience:
  • Required: At least 6 months of phone-based customer service experience in a fast-paced, high-volume environment, such as a call center.
  • Preferred: 1+ years of experience in home health or home medical equipment settings.

Requirements

• Coordinate referrals, enter patient information in the EMR, and ensure smooth patient admissions.
• Serve as a liaison with hospitals, physicians, patients, and families to support timely, high-quality care.
• Manage provider orders, documentation, and compliance to maintain accurate, efficient admissions.
• Work in a fast-paced, collaborative environment where your contributions directly impact patient care.
