Role Description

We are seeking an individual freelancer (not an agency or BPO) based in Haiti to join our team as a Care Coordination Specialist. The specialist will support Haitian Creole-speaking members and caregivers through outbound calls, intake follow-ups, and new referral onboarding. This role is essential in reinforcing program expectations, confirming daily visits, addressing no-shows, and performing compliance follow-ups for at-risk members. All interactions must be documented in the CRM system, with SMS or WhatsApp follow-ups sent as appropriate.

Key Responsibilities

• Conduct outbound calls to Haitian Creole-speaking members and caregivers
• Assist with intake follow-ups and new referral onboarding
• Reinforce program expectations and visit schedules
• Confirm daily visits and address no-shows
• Perform compliance follow-ups for at-risk members
• Document all interactions in the CRM system
• Send SMS or WhatsApp follow-ups when appropriate

Qualifications

• Fluent in Haitian Creole, French, and English (verbal and written)
• Prior experience in call center, customer service, or patient coordination roles
• Comfortable with high call volumes and structured workflows
• Strong communication and documentation skills

Preferred Qualifications

• Experience in healthcare, home care, or Medicaid-related programs
• Familiarity with CRM systems
• Experience working with remote teams
