Role Description

As a Referral Coordinator, you will play a key role by:

• Scheduling referred patient appointments.
• Tracking and scheduling circulation rides.
• Answering questions from community partners (including jails, emergency departments, treatment facilities, and others).

Additional responsibilities include:

• Receiving inbound referrals from community partners in the company's CRM and EHR, via phone, chat, or email; processing and scheduling, if appropriate.
• Documenting necessary information in patient charts.
• Gathering new patient demographic and insurance information.
• Following up on patient inquiries.
• Developing trusted relationships with community partners.
• Understanding nuances of community partner referrals and providing accurate responses.
• Transferring phone calls, providing phone messages, and escalating to appropriate staff.
• Routing Ideal Option patient inmate prescription refill requests to prescribing provider.
• Using problem-solving skills to implement solutions and escalate unresolved problems.
• Representing Ideal Option professionally and with a positive attitude.
• Participating in continuous quality improvement activities.
• Complying with national Protected Health Information laws including HIPAA and 42 CFR Part 2.
• Working on-call for one week once every 4-5 weeks.
• Performing other projects and duties as assigned.

Qualifications

• Excellent customer service skills.
• Typing speed of 60 words per minute or more.
• General knowledge regarding the use of office equipment.
• Effective verbal and written communication skills, especially conveying complex information in an understandable manner.
• Proficient in CRM and EHR systems.
• Ability to multitask, learn quickly, and work in a fast-paced environment.
• Ability to work independently and as a team member.
• Ability to establish and grow strong relationships.

Requirements

• Must reside in the Eugene, OR area.
• High School Diploma or GED.
• One year of customer service experience.
• Preferably one year or more of experience in office work.
• Ability to sit for long periods of time.
• Experience with personal computers, word processing, and electronic spreadsheets.
• Remote work from home.
• Must have laptop or computer and will be provided with a company phone for calls.
• Periods of stress may occur.
• Must be in a quiet and private environment.
• Experience with call center.
• Experience with Community Organizations strongly desired.
