About Homebase Medical

Homebase Medical is SCAN Group’s in-home care delivery platform — providing comprehensive, coordinated care for seniors with complex, chronic, and advanced conditions. Through services such as Annual Wellness Visits, transitional and palliative care, and longitudinal medical management, Homebase bridges critical gaps in the care continuum.

By integrating technology, workforce optimization, and compassionate service, Homebase enables SCAN to achieve its mission of keeping seniors healthy and independent while managing total cost of care and improving quality performance across Non-Delegated Risk (NDR), shared-risk, and direct-contracting partnerships.

Your Qualifications

Master’s Degree in Healthcare Administration or similar field.

Tenured professional who has a minimum 5+ years’ experience as a General Manager, Operations or Clinical leader, or other senior executive role, with a strong preference coming from the payer or provider industries.

10+ years in healthcare delivery or managed care, with proven track record of organizational transformation and deep understanding of value-based care economics and risk-based arrangements.

Demonstrated experience in executive level management, development of leadership teams, and driving culture change in clinical organizations.

Demonstrated B2B2C growth experience and proven ability to scale a company or function within a company.
