Role Description

As a Lead Director of Population Health, you will define the clinical program requirements, workflow intent, and success criteria that drive technology enablement for complex care coordination programs directly impacting high-risk patient outcomes. This role is the connective tissue between what clinical leadership needs and what our systems can deliver.

• Own definition of clinical requirements for key OSH initiatives keeping patients healthy and out of the hospital.
• Partner with Informatics and Product from design through go-live and ongoing optimization.
• Develop prioritized clinical requirements and use cases to inform Epic and non-Epic technology roadmaps.
• Translate clinical program logic and workflows into clear, actionable requirements for technical partners.
• Drive the clinical-operations side of complex, multi-team launches.
• Validate that delivered features meet clinical and workflow requirements.
• Develop frameworks to measure program success across a wide range of stakeholders.
• Monitor program performance data and partner with Analytics to resolve issues quickly.
• Represent Population Health in cross-functional forums.
• Lead, develop, and retain a high-performing team.
• Develop and maintain executive-ready briefings and strategy documents.
• Proactively identify gaps between strategic priorities and current-state capabilities.

Qualifications

• 10-15 years in healthcare program management, population health, or care delivery operations.
• Deep working fluency with EHR systems (Epic strongly preferred).
• Demonstrated ability to earn trust and influence technical and build teams.
• Clinical + operational bilingualism.
• Exceptional written communication skills.
• Proven ability to drive the clinical-operations workstream of high-stakes go-lives.
• Strong analytical instincts.
• People leadership experience with a track record of developing early-career talent.
• High bias toward action; thrives in ambiguity.
• Ability to flag risk clearly and early.

Requirements

• Bachelor's degree required.
• Graduate degree preferred but not required (MBA, MPH).
