[Hiring] HIM Chart Deficiency Specialist @UnityPoint Health

Role Description

We’re seeking a detail-oriented professional to perform quantitative analysis and ensure the completeness and accuracy of medical records. As the HIM Chart Deficiency Specialist, you will review clinical documentation and verify that all required elements are present, helping prevent compliance risks and supporting high-quality patient care.

As a key contributor within the HIM department, you will help safeguard accurate reimbursement, support survey readiness, and maintain documentation standards across the organization.

• Responsible for reviewing content, completion, timeliness and accuracy of medical record documentation to ensure the quality of documents
• Perform quantitative chart analysis on patient records, as defined by department management
• Ensure that the documentation meets the requirements per State, Federal, DNV, The Joint Commission, and Medical Staff rules and regulations as assigned
• Assign record deficiencies for provider completion
• Provide ongoing communication to appropriate personnel for education to the provider on the documentation completion process
• Collaborate with other HIM functional areas to investigate and resolve issues related to chart deficiencies
• Responsible for routing scanned reports for documentation corrections
• Maintain quality and productivity expectations

Qualifications

• High School diploma/GED required
• Associates degree in Health Science or Bachelors degree in HIM preferred
• One year of experience with an electronic health record or hospital/medical setting
• Previous medical clerical/office experience or formal training in the Health Information Technology field
• RHIT or RHIA certifications preferred
