Role Description

The Health Team Coordinator plays a pivotal role in ensuring the smooth day-to-day operation of the Confident Futures service. Acting as the central point of coordination for participants and the multidisciplinary team, the postholder supports:

  • Participant engagement
  • Participant coordination
  • Appointment management
  • Operational administration
  • Service facilitation

The Health Team Coordinator is often the first point of contact for participants and plays an important role in building confidence, maintaining engagement and reducing barriers to participation throughout the programme.

Key Responsibilities

  • Coordinate the day-to-day operational support required by the Health and Employment Specialist team.
  • Act as a friendly and responsive point of contact for participants.
  • Support appointment arrangements and respond to routine enquiries.
  • Contact participants who have missed appointments to encourage re-engagement.
  • Support the efficient operation of the Confident Futures service by coordinating participant allocations and scheduling appointments.
  • Maintain well-managed caseloads and monitor diary capacity.
  • Assist with the coordination of Warm Handovers.
  • Maintain accurate participant records within the case management system.
  • Support the collection of participant feedback and outcome measures.
  • Provide practical support for the delivery of virtual wellbeing sessions and community-based activities.
  • Maintain high standards of confidentiality, information governance and data quality.
  • Escalate safeguarding or participant welfare concerns promptly.
  • Contribute to a culture of continuous improvement.

Qualifications

  • Educated to Level 3 or above in Business Administration, Health & Social Care, Customer Service, Public Services or another relevant discipline, or equivalent demonstrable experience.
  • Experience providing administrative, coordination or participant support within health, wellbeing, employability, social care or community services.
  • Experience managing appointments, diaries, referrals or caseload administration within a fast-paced environment.
  • Strong organisational and prioritisation skills.
  • Excellent communication and interpersonal skills.
  • Experience maintaining accurate records and case management systems.
  • Good digital literacy, including Microsoft Office, Microsoft Teams and CRM or case management systems.
  • Understanding of safeguarding, confidentiality, GDPR and professional boundaries.
  • Ability to work independently whilst contributing effectively as part of a multidisciplinary team.
  • Commitment to delivering high-quality, person-centred services aligned to the Social Model of Disability.

Requirements

  • Level 4 or above qualification in Health & Social Care, Business Administration, Public Health, Wellbeing or a related subject (Desirable).
  • Experience working within employability, vocational rehabilitation, public health or DWP-funded programmes (Desirable).
  • Experience coordinating virtual and face-to-face appointments, workshops or group sessions (Desirable).
  • Experience supporting participant engagement and reducing non-attendance (Desirable).
  • Knowledge of the Social Model of Disability and strengths-based approaches (Desirable).
  • Experience using outcome measurement tools such as the Wellbeing Star, SWEMWBS or similar wellbeing frameworks (Desirable).
  • Experience working collaboratively with healthcare professionals and community organisations (Desirable).
  • Understanding of motivational interviewing, behaviour change principles or trauma-informed approaches (Desirable).
  • Experience supporting quality assurance, audit activity and contractual compliance (Desirable).
