Role Description

This is a full-time role operating from central or eastern regions of the United States during standard business hours, Monday - Friday (from your home office remotely) with some travel in the United States for company meetings (this is not a flexible schedule opportunity).

This role handles sensitive information; work must be performed in a private and secure setting (with a door) requiring reliable internet and phone connectivity. The candidate must reside in the Central or Eastern time zones/region of the United States. This role does not offer immigration visa sponsorship.

The Director of Care Transitions and Strategic Partnerships is responsible for the strategic growth, operational performance, analytics, and continued development of the organization's FastPAC and FastPAC Plus programs. The Director will oversee the programs across the continuum of care, ensuring effective identification and tracking of participating patients, coordination between hospitals and post-acute providers, communication with appropriate clinical and community partners, and successful transition of patients through the care continuum.

A primary responsibility of this position is to demonstrate the value and outcomes of FastPAC through meaningful data and executive-level reporting. The Director will analyze program performance, identify trends and opportunities, and regularly present results to senior executives, hospital leadership, payer partners, physician groups, post-acute organizations, and other strategic partners.

Key Responsibilities

  • FastPAC & FastPAC Plus Program Leadership:
    • Provide overall operational leadership for FastPAC and FastPAC Plus and ensure consistent execution of the programs across participating markets and facilities.
    • Maintain standardized program expectations while recognizing the need to adapt workflows based on hospital, post-acute, payer, provider, and market requirements.
    • Ensure participating teams understand program objectives, responsibilities, communication expectations, and performance standards.
  • Program Growth & Expansion:
    • Develop and execute strategies to expand FastPAC and FastPAC Plus across new hospitals, skilled nursing facilities, markets, physician practices, payer relationships, and other strategic partnerships.
    • Evaluate new opportunities based on patient population, referral patterns, organizational presence, strategic alignment, operational feasibility, and potential clinical and financial value.
    • Partner with Operations, Business Development, Clinical Leadership, and executive leadership to support program expansion and implementation.
  • Care Continuum Integration:
    • Ensure FastPAC effectively connects the patient's journey across the acute and post-acute continuum.
    • Work collaboratively with hospital teams, Medrina providers, care coordinators, SNFs, physician practices, Care Management, Population Health, and other partners to improve communication and visibility as patients transition between care settings.
    • Identify breakdowns in communication or continuity and develop solutions that strengthen the transition process.
  • FastPAC Plus Development:
    • Lead the continued development and differentiation of FastPAC Plus as an enhanced care-transition and patient-engagement model.
    • Identify opportunities to incorporate additional services, technology, clinical support, Care Management, Population Health, or other capabilities that increase the value of the program to patients and strategic partners.
    • Work with executive and clinical leadership to define the evolving FastPAC Plus model and demonstrate the incremental value of the enhanced program.
  • Data Analytics & Outcomes:
    • Develop a strong analytical framework for measuring the clinical, operational, and financial performance of FastPAC and FastPAC Plus.
    • Analyze program performance and identify trends related to patient transitions, utilization, readmissions, emergency department utilization, length of stay, patient capture, post-acute placement, follow-up, and other agreed-upon performance measures.
    • Partner with Data Analytics, Finance, Operations, and Clinical Leadership to validate data and ensure program results are accurate, meaningful, and actionable.
    • Use data to identify opportunities to improve workflows, patient outcomes, partner performance, and program growth.
  • Executive Reporting & Presentations:
    • Develop high-quality executive reports, dashboards, presentations, and program summaries demonstrating FastPAC and FastPAC Plus performance and outcomes.
    • Regularly present findings to various stakeholders including hospital executives, health system leadership, payer partners, and internal executive leadership.
    • Translate complex operational and clinical data into clear executive-level messaging that demonstrates the program's value and identifies opportunities for improvement or expansion.
  • Strategic Partner Relationships:
    • Develop and maintain strong relationships with hospital, post-acute, payer, physician, and other strategic partners participating in FastPAC.
    • Serve as a key operational point of contact for the program and ensure partner concerns, opportunities, and performance issues are appropriately addressed.
    • Participate in executive business reviews and strategic partnership meetings and provide program updates and recommendations.
  • Operational Performance:
    • Monitor FastPAC performance across participating hospitals, facilities, markets, and partners.
    • Identify variations in performance and work with operational leaders to determine root causes and corrective actions.
    • Establish accountability for program execution while ensuring operational processes remain efficient and scalable as FastPAC expands.
  • Quality & Continuous Improvement:
    • Use program data and partner feedback to continually improve FastPAC and FastPAC Plus.
    • Identify opportunities to strengthen patient identification, communication, transitions, post-acute coordination, follow-up, and overall continuity of care.
    • Evaluate successful practices across markets and develop standardized approaches that can be replicated throughout the organization.
