Role Description

Vupico is looking for a passionate and experienced Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and providing support to individuals in need of social services. Your role will involve:

• Conducting comprehensive assessments to identify client needs and develop appropriate treatment plans
• Linking clients to resources and services such as housing, healthcare, education, and employment
• Monitoring client progress and providing ongoing support and guidance
• Advocating for clients and collaborating with other professionals, agencies, and organizations
• Maintaining accurate and up-to-date client records and reports
• Providing crisis intervention and addressing any problems or issues that may arise
• Educating clients and their families on community resources and support networks
• Developing and facilitating support groups and educational workshops

Qualifications

• Bachelor's or Master's degree in social work, psychology, counseling, or related field
• Minimum of 2 years of experience in case management or related field
• Knowledge of social services and community resources
• Excellent communication, organization, and interpersonal skills
• Ability to work independently and as part of a team
• Experience working with diverse populations, such as low-income families, refugees, and individuals with disabilities
• Proficient in Microsoft Office and database management systems
• Bilingual in English and Spanish is a plus
