Role Description

As a Care Coordinator I, you will help support the overall health, safety, and well-being of assigned members while coordinating services, navigating complex systems, advocating for individuals and families, and connecting members to the resources they need.

The ideal candidate brings experience working with individuals with intellectual and developmental disabilities (IDD), behavioral health needs, substance use disorders, Medicaid services, waiver programs, foster care, community-based services, or other complex populations.

You will be responsible for:

• Coordinating services and supporting the overall health, safety, and well-being of assigned members.
• Working with members, families, guardians, providers, state agencies, community organizations, and other members of the care team.
• Maintaining required documentation and monitoring member needs and performance requirements.
• Effectively managing a field-based schedule while balancing member needs, appointments, documentation requirements, and regulatory deadlines.

Qualifications

• Bachelor’s degree in social work, psychology, sociology, another social science, or a health-related field; or
• Two years of college coursework in social work, psychology, sociology, another social science, or a health-related field plus one year of experience working with individuals with developmental, intellectual, and/or behavioral health disabilities and/or substance use disorders; or
• An equivalent combination of education and relevant professional experience.

Requirements

• Must possess a valid Arkansas Class D (Non-Commercial Vehicle) Driver’s License before employment and maintain licensure throughout employment.
• Must have access to a properly licensed, insured, and reliable automobile for assigned business travel.
• Must provide proof of current, valid automobile insurance before employment and thereafter upon request.
• Must reliably travel to member homes, provider offices, community locations, and meetings as required.
• This position requires one or more pre-employment screenings, which may include criminal background investigation, drug and alcohol screening, motor vehicle report, child maltreatment registry check, adult maltreatment registry check, sex offender status check, and verification of education, certifications, and licensure.
