[Hiring] Program Manager, Mobile and Virtual Care @Adams Health

Role Description

The Program Manager, Mobile and Virtual Care leads the planning, implementation, coordination, and performance monitoring of approved mobile and virtual care initiatives. This role has primary grant-charged responsibility for the Community Health Worker-supported mobile care model and establishes an accountable operating structure to support effective program delivery.

• Coordinates cross-functional activities
• Monitors program scope, timelines, budgets, documentation, and performance measures
• Supports compliance with RHTP grant requirements
• Works collaboratively with clinical, operational, community, and external partners
• Aims to improve rural access to healthcare through sustainable mobile and virtual care solutions

This is a grant-funded, time-limited position dependent on continued RHTP/GROW grant funding and organizational need. Future funding is not guaranteed.

Qualifications

• Bachelor’s degree in nursing, healthcare administration, public health, business, health informatics, or a related field required
• Master’s degree preferred
• Relevant professional licensure must be maintained, when applicable to assigned duties

Requirements

• 20 hours per week - First Shift
• Experience in healthcare operations, project or program management, population health, mobile care, telehealth, or a related field required
• Experience working in rural healthcare and/or with grant-funded program implementation preferred
