[Hiring] Visit Facilitator @Fairview Health Services

Role Description

Fairview has an immediate opening for an Virtual Visit Facilitator to join our team. This is a 1.0 FTE (80 hours per two weeks pay period), benefits-eligible opening.

Visit Facilitators are a valuable member of our care team and are responsible for supporting in-person clinic visits, including:

• Communicating with patients prior to their visit
• Helping guide patients through the care experience
• Gathering pertinent medical information from the patient
• Documenting the information into the patient's medical record
• Assisting with tasks that support the effectiveness and efficiency of the team
• Scheduling and coordinating appointments as needed
• Preparing schedules and other delegated duties as assigned

Clinic Hours of Operation are Monday–Thursday: 7:00AM–6:00PM and Friday: 7:00AM–5:00PM.

Responsibilities

• Ensures all pertinent medical records have been received and are available for the visit
• Updates Care Everywhere as requested
• Collects data and medical device downloads as needed
• Monitors and completes registration errors within the work queue
• Uses technology proficiently to identify and satisfy patient appointment needs for a clean claim
• Acquires and verifies patient demographic, guarantor, and insurance information
• Authorizes and collects copayment and prepayment as needed
• Coordinates patient support with interpreter services as appropriate
• Reviews and updates allergies, medications, health assessments and/or questionnaires, and patient-reported vitals
• Provides technology support to both patient and provider to ensure video and audio are functioning
• Communicates with providers and other clinical staff to support the virtual visit experience

Qualifications

• 1 year of customer service experience
• Familiarity with medical terminology
• Previously worked in a healthcare setting
• Telephonic customer experience
