Role Description
Jefferson Primary Care is currently accepting candidates for the position of Supplemental Access physician, to provide telehealth-based care in service to patients of the primary care network. This position is for a full-time physician to provide telehealth services. It is a unique opportunity to complete outreach to patients who will benefit from a primary care visit and reconnection to a medical home.
Please note:
While this position is remote, there is a strong preference for candidates who reside in PA, NJ or DE.
The Supplemental Access physician will work closely with primary care and population health teams to connect with patients who require same or next day primary care access, have been out of care longer than recommended intervals and/or who require care at critical touchpoints such as transitions from hospital discharge. After completing the visit, the physician will then help the patient reintegrate into a Jefferson Primary Care practice.

Provide patient-centered care with a commitment to excellence in patient experience.

Complete annual screenings and/or annual wellness visits with patients as assigned.

Complete additional required screenings and care deliverables such as social screenings, advance care plans and ordering of preventive screenings.

Assess chronic disease management needs and assist patients in improving their disease management as a bridge to reconnection to the continuity primary care provider.

Communicate with continuity primary care providers as a key partner in re-engaging and caring for patients in a team-based model.

Connect patients to medical home and specialist services as indicated.

Apply a trauma-informed, health equity lens in clinical service.
Qualifications

Qualified applicants will hold a MD or DO degree, or equivalent.

Board certified, or eligible, in Family Medicine or Internal Medicine.

3+ years of primary care experience.

Licensure in both PA and NJ required, or ability to obtain by start date.

Demonstrated commitment to health equity, to excellence in care including for medically and socially vulnerable populations, and to optimal patient experience.
