[Hiring] Virtual Functional Medicine - Nurse Practitioner @Aligned Modern Health

Role Description

We are seeking outstanding Nurse Practitioners to join our Functional Medicine practice.

• Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.
• Develop personalized treatment plans based on patients' needs and medical conditions.
• Prescribe nutraceuticals and/or medications and monitor their effectiveness.
• Collaborate with other healthcare professionals to coordinate patient care.
• Provide counseling and education to patients on disease prevention and healthy lifestyle choices.
• Maintain accurate and up-to-date patient records using electronic health record systems.
• Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative, and lifestyle medicine.

Qualifications

• At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs.
• Experience in functional and integrative medicine settings preferred.
• NP/APRN – with a concentration in Functional Medicine, Hormone Health, Anti-Aging, or Lifestyle Medicine.
• Providers based in FL, MI, TN, and TX are preferred.
• Must have (or be able to obtain) an IL APRN license and any remote location determined.
• Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers.
• Energetic and outgoing personality with a desire to help people.
• Patient first attitude and ability to consistently provide a 5-star level of service.
• Knowledge of HIPAA regulations and ability to maintain patient confidentiality.

Requirements

• Active Illinois APRN license required to start employment.
