Role Description

Optum Home & Community Care is creating something new in health care, uniting industry-leading solutions to build an integrated care model that addresses an individual’s physical, mental, and social needs. As a team member, you will help patients navigate the health care system and connect them to key support services.

The Urgent Care Registered Nurse (RN) is responsible for:

• Providing telephonic care and direction to patients/members during various hours.
• Delivering medical care services to enrolled patients/members in Optum programs.
• Working in close collaboration with team members and supporting multiple providers.
• Understanding and supporting the Optum Models of Care.

This is a home-based role requiring various shift coverage, including weekdays, weeknights, weekends, and holidays.

Qualifications

• Current, unrestricted RN compact license.
• Additional licensure in assigned states must be obtained within 6 months of hire.
• 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management position.
• Dedicated space for home office setup.
• Access to high-speed internet services.
• Computer/typing proficiency to enter and retrieve data in electronic clinical records.
• Proficient with Microsoft Word, Outlook, and Excel.
• Solid problem-solving skills.
• Ability to communicate complex or technical information clearly.
• Ability to flex with change and perform positively in a production-driven environment.
• Ability to work scheduled shifts in accordance with scheduling policies.

Requirements

• Must attend and complete all mandatory education and MyLearning training requirements.
• Participate in the onboarding of new clinical staff under the direction of the CSM.
• Document all clinical information, telephonic assessments, and activities in the appropriate documentation tool.
• Responsible for escalating appropriate calls to the Optum APC.
• Document all interactions within the appropriate electronic medical record.
