Role Description

We are seeking a highly skilled and collaborative Primary Care Physician to join our growing telehealth team supporting patients in skilled nursing facilities (SNFs). This is a unique opportunity to deliver high-impact care remotely while partnering closely with on-site clinical teams and leveraging innovative diagnostic technology.

• Conduct on-demand telemedicine visits.
• Interpret diagnostic testing results.
• Guide care plans in collaboration with facility staff.
• Perform structured evaluations of nurse practitioners to ensure high-quality, patient-centered care.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics.
• Licensed or willing to obtain licensure in New Jersey and Rhode Island; Connecticut licensure may be required in the future.
• Experience working with geriatric populations and/or in skilled nursing facilities strongly preferred.
• Prior telehealth experience is a plus.
• Demonstrated ability to collaborate effectively with interdisciplinary teams.
• Strong clinical judgment and comfort interpreting diagnostic data remotely.

Requirements

• Conduct remote primary care and consultative visits with patients in skilled nursing facilities via telemedicine.
• Review and interpret diagnostic testing results obtained through mobile clinical technology (e.g., diagnostic carts).
• Develop and communicate treatment recommendations in collaboration with on-site medical staff.
• Partner with interdisciplinary care teams to ensure continuity and quality of care.
• Document all patient encounters and recommendations in a timely and compliant manner.
• Perform 5–10 structured clinical assessments per day (on designated days) for nurse practitioners.
• Conduct comprehensive chart reviews and clinical audits.
• Observe patient interactions and care delivery (virtually).
• Provide constructive feedback, coaching, and guidance to support clinical excellence.
• Ensure adherence to evidence-based practices, organizational standards, and regulatory requirements.
