Role Description
The Certified Diabetes Care and Education Specialist (CDCES) will work directly with patients to provide diabetes education and support with the goal of improving adherence and health outcomes. This remote position is responsible for consistently meeting program metrics while also meeting quality requirements. The CDCES position is a part-time and independent contractor role.

Provide diabetes education and management support to improve glycemic control, treatment adherence, safety, and health outcomes.

Provide coverage for scheduled shifts, delivering 24/7 remote device monitoring across connected and manual data sources to triage and educate members in real time or through scheduled support sessions, while supporting medication management, safety, and goal achievement.

Pay differential for overnight remote patient monitoring shifts.

Quickly establish rapport and therapeutic alliances via phone, video telehealth platform, and digital communication with a diverse population of members with T1, T2, and LADA diabetes.

Deliver medication management, including adjustments within protocol and/or recommendations to the provider.

Remote device monitoring for connected and manual device data to inform in the moment or scheduled support sessions, medication management, safety, and goal achievement.

Drive measurable improvements in diabetes glycemic control and support comprehensive management of coexisting comorbid conditions.

Produce behavior change among members using motivational interviewing techniques.

Present member case studies in team meetings.

Demonstrate consistent performance across responsiveness, documentation, scheduling, and quality requirements.

Qualifications

Must have a current certification as a Certified Diabetes Care and Education Specialist from the National Certification Board for Diabetes Educators (NCBDE).

Must be licensed if required by the state you live in.

Comfort working in alert-driven and reactive clinical care models and the ability to assess clinical risk and identify when escalation of care is required.

Working knowledge of diabetes management technologies, including continuous glucose monitoring (CGM), automated insulin delivery (AID) systems, continuous subcutaneous insulin infusion (CSII), and connected insulin pens.

Demonstrated knowledge of diabetes medication management, including supervised experience with medication adjustments.

Demonstrated ability to receive constructive feedback and then incorporate it into patient calls to drive performance.

Proven experience utilizing motivational interviewing to provide positive behavior change.

Must have a home office for private member conversation along with high-speed internet.

Able to work independently and manage time efficiently.

Must be comfortable utilizing advanced technology (Electronic Medical Records, Microsoft Office, scripting applications, device data, etc.).

Demonstrated ability to recognize member needs and adapt communication techniques to best serve a diverse patient population.

Must have a flexible schedule and be able to commit to a minimum of 10 hours per week to accommodate members.
