As a senior member of the Care Team, the Senior Clinical Care Navigator serves as the lead architect for the most complex patient journeys and multi-system coordination needs.

Manage complex cardiology caseloads, guiding patients from diagnosis through recovery.

Represent the Cardiology Service Line, presenting critical data to leadership and cross-functional partners.

Take ownership of the clinical and operational success of your specific service line.

Use technical writing skills to transform complex workflows into clear, scalable Standard Operating Procedures (SOPs).

Influence care models, setting the gold standard for navigation workflows.

Act as the bridge between COE clinical experts and clinical operations teams.

Partner with cross-functional teams to support internal tools and processes.
Qualifications

Registered Nurse with a BSN and 8+ years of progressive experience in cardiology.

Technical savvy to collect, report, and present program metrics.

Ability to work seamlessly in interdisciplinary environments.

Proven ability to manage complex workflows and high-impact responsibilities.

Consistent track record of reliability, growth, and increasing scope of responsibility.

Commitment to providing the highest level of service through empathy, professionalism, and integrity.

Ability to handle confidential information and adhere to HIPAA requirements.

Comfortable interacting through various tech platforms and learning new software quickly.
Requirements

Bachelor of Science in Nursing (BSN) from an accredited School of Nursing is required.

Minimum of 8+ years of progressive clinical experience in Cardiology is preferred.

Current and unrestricted Registered Nurse licensure.

Nice to have: Spanish Speaking preferred.

Nice to have: Board certification in Cardiac-Vascular Nursing (RN-BC) or as a Care Coordination and Transition Management (CCCTM) professional is highly recommended.
