In this role you will support the South Team covering mostly Summit and Eastern Medina counties:

• Medina
• Brunswick
• Wadsworth
• Lodi
• Stow
• Akron
• Cuyahoga Falls
• Hudson
• Fairlawn
• Richfield

You will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will:

• Provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting.
• Work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules.
• Receive a generous in-depth orientation tailored to each individual and their experience level.

This position is eligible for student loan repayment.

Caregivers in this role will work 8:00am to 5:00pm or 8:00am to 6:00pm and can be 8- or 10-hr shifts. On call, weekend and holiday requirements.

A caregiver who excels in this role will:

• Provide direct nursing care.
• Perform accurate and complete admission assessments and reassessments within a specified time frame.
• Assess patients for safety risks, abuse and neglect.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Assess, document, prioritize and manage patients’ pain.

Qualifications

• Graduate from an accredited school of Professional Nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
• Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
• One year of current experience as an RN
• One year of experience in an acute Medical/Surgical or Home Care setting

Requirements

• Bachelor’s of Science in Nursing (BSN)
• Familiarity with complex medical treatments
• Experience working in homecare, med/surg, ICU and/or ED areas
