Role Description

Make a difference in the lives of patients — from the comfort of your home. As a Care Manager, you’ll provide telephonic education and coaching to patients managing chronic conditions such as diabetes, hypertension, and obesity. You’ll be part of a supportive program designed to improve patient outcomes and lower healthcare costs — and you’ll be trained in health coaching methods to help patients make positive lifestyle changes.

Key Responsibilities

• Identify and engage patients who could benefit from coaching.
• Conduct telephonic outreach to explain the program and encourage enrollment.
• Educate and motivate patients toward healthier behaviors by using motivational interviewing and behavior change science.
• Document interactions clearly and accurately in our system.
• Triage and refer patients to appropriate services as needed.

Qualifications

• Active, unrestricted RN license in the US.
• Previous experience in an employee wellness or health promotion role preferred.
• Spanish-speaking ability is a plus.
• Minimum internet speed requirement: 25 Mbps download / 25 Mbps upload.

• Ability to follow detailed procedures and work independently.
• Effective decision-making and problem-solving abilities.
• Proficiency with Microsoft Outlook and Windows-based software.
• Strong multitasking and organizational skills.
