Role Description

The Clinical Nurse Care Coordinator collaborates with clinic team members to provide evidence-based patient-centered care to patients, helping them to stay up to date on their care plans and related goals towards improving health.

• Focus on high-risk medically complex patients.
• Work with patients on specific disease registries.
• Assist patients with disorders or chronic physical or intellectual disabilities.
• Support patients in care transitions (e.g., pediatric to adult care, recent inpatient admissions and discharges, ED follow-up, care facility discharges).
• Act as a liaison with patient’s family members and other agencies to facilitate improved communication.
• Stay up to date on standards, assisting with programs and processes.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN accredited nursing program.
• Current Ohio Registered Nurse license.
• CPR/BLS certified.
• Knowledge of triage procedures along with clinic, physician office and/or urgent care experience.
• Ability to maintain quality, safety, and/or infection control standards.
• Knowledge of appropriate procedures and standards for the administration of medications and patient care aids.
• Ability to educate and clearly communicate with patients and/or families regarding the nature of disease and to provide instruction on proper care and treatment.
• Ability to convey information both in writing and verbally.
• High level of interpersonal skills to handle sensitive and confidential situations.
• Position continually requires demonstrated poise, tact and diplomacy.
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions.

Requirements

• Current Ohio Registered Nurse license.
• CPR/BLS certified.

Pay Range

USD $32.85 - USD $48.55 /Hr.
