Role Description
Kaiser Permanente-Washington Permanente Medical Group is seeking a full-time 1.0 FTE Primary Care Physician Assistant to join our Quality, Documentation & Reporting Team.
The VC3 (Virtual Care for Chronic Conditions) Clinician will work to ensure quality care for identified WPMG patients utilizing virtual encounters to address conditions and close care gaps in a patient-centered manner. Whenever possible, they will preview, update, and modify the health record in HealthConnect to optimize the chart for their virtual encounters and perform PC3 (Patient Centered Charting and Communication) chart reviews between virtual visits. The work completed by the VC3 Clinicians will be monitored through proficiency metrics and chart audits to ensure quality of documentation. VC3 Clinicians will report to the Assistant Medical Director of Documentation and Reporting.
Principal Duties & Responsibilities

Conduct on-demand virtual visits for adult members in conjunction with existing Specialty Care encounters or when the patient is onsite for other care needs.

Be available during normal clinic hours Monday through Friday.

Conduct chart reviews to ensure completeness and accuracy of the medical record according to PC3 Clinician Reviewer standards.

Provide accurate and complete documentation of disease state management to address conditions.

Provide expert integrated care and diagnostic services in the virtual care setting.

Work remotely in compliance with all WPMG & KPWA remote work policies.

Work closely with Primary Care, Specialty Care, and THMC to improve care coordination for our members.

Perform PC3 chart reviews between virtual visits according to PC3 standards.

Maintain flexibility in staffing to ensure coverage for VC3 department.

Maintain an appropriate work from home environment including professional background and privacy.

Work effectively in an inter-disciplinary team collaborating with care teams.

Analyze reports, test results, medical records, and examinations to diagnose conditions and propose treatment options.

Review/update the medical record including current medications, allergies, medical/surgery history, etc.

Provide advice to patients for lifestyle and diet changes.

Adhere to departmental policies, procedures, and ongoing quality improvement objectives.

Maintain patient confidentiality and comply with all federal and state health information privacy laws.

Record complete, timely electronic medical records that meet documentation and coding standards.

Be available to work in at least half day increments (0.1 FTE).
Qualifications
Requirements

Minimum: VC3 Clinician must have proficiency in using HealthConnect.

Must routinely use the ambulatory instance of HealthConnect for clinical documentation.

Must be in one of the following specialties: Family Medicine, Internal Medicine, or Pediatrics or similar broad scope subspecialty.

Must be in good clinical standing with support from their local leader (AMD) to pursue this work.

Must maintain good medical standing, active licensure, and board certification in their specialty.

Work remotely in compliance with all WPMG & KPWA remote work policies.

Currently work in one of the following settings: Primary Care or other comprehensive specialty service line.
Licenses, Certifications or Other Essential Qualifications
